Application fee of Rs.1000/- is to be paid online only. Pay the fees at www.rguhs.ac.in - online payment gateway, prior to filling the online application. Read Application fees payment guide for further details.

Attested copies of following certificates should be submitted along with the printed online filled application form

a. Pass certificate / Degree certificate

b. Marks cards of all university examinations passed (First to final year of the course)

c. Attempt Certificate issued by the Principal

d. Certificate regarding recognition of the college by the respective Councils/regulatory bodies

e. Completion of rotating internship certificate from a recognized college where applicable

f. Registration by the respective state council wherever applicable

g. Proof of SC/ST or Category as the case may be

Print out of online filled application form and attested copies of documents should be submitted by post or in person to The Registrar, Rajiv Gandhi University of Health Sciences, Karnataka, 4^th T block, Jayanagar, Bangalore - 560 041 on or before 10th October 2019, 5 pm.

The envelope must be superscribed as MPH, PGET - 2019.
